To add a trust as an HSA Beneficiary for your Health Savings Account, follow these instructions:
1. Log in to your Ameriflex portal.
2. From the homepage, click the More tab at the top of the screen. From the drop-down, select Beneficiaries.
4. Select which Beneficiary type you would like to add, such as Primary or Contingent.
5. Once you select the Beneficiary type, a pop-up box will appear for you to enter the Beneficiary information. Under the Relationship box, select the drop-down and click Trust.
6. Fill out the remaining information and click Save.
