Following an employee's termination, the responsibility for managing their benefits transitions to the plan administrator or broker, particularly concerning COBRA continuation. This document provides a step-by-step guide detailing the process, including notification to Ameriflex, participant notification, and the subsequent actions based on the participant's election or waiver of coverage. It also emphasizes the importance of accurate carrier enrollment contacts to prevent delays in coverage reinstatement.

Here are the steps of the Qualified Beneficiary process after an employee terminates:

  1. Employee Terminates - Upon termination, the plan administrator/broker will remove the employee from active employee benefits. 
  2. Notify Ameriflex - The Plan administrator/broker will enter the qualified beneficiary into the Ameriflex portal.
  3. Ameriflex Notifies Qualified Beneficiary - Once entered into the portal, Ameriflex will generate the COBRA Continuation Election Notice overnight and mail it the following business day.
  4. Qualified Beneficiary Elects/Waives Coverage

If the Qualified Beneficiary Elects Coverage: The Qualified Beneficiary notifies Ameriflex of their election and makes the full initial premium payment to Ameriflex. Ameriflex will then email a carrier notice of reinstatement to the enrollment contact provided for each of the specific plans elected. Note: The carrier enrollment contact listed during plan setup must ensure the Qualified Beneficiary is enrolled in the coverage(s) elected. If the broker or plan administrator is listed as the carrier enrollment contact, they will be responsible for reaching out to the carrier once they receive the emailed notice of reinstatement. We recommend that you provide Ameriflex with direct carrier contacts to ensure no delays in coverage.

If the Qualified Beneficiary Waives Coverage: No other action is required as the Qualified Beneficiary was already terminated from active employee benefits by the plan administrator/broker.

If the Qualified Beneficiary Doesn't Respond: No other action is required as the Qualified Beneficiary was already terminated from active employee benefits by the plan administrator/broker.
