Submit a Transaction Reversal Request

Did your employee's funds come out of the wrong plan or plan year? Do you need to reverse a transaction? In this article, we will walk you through how to handle a transaction that was applied to the wrong plan or wrong plan year.

When a member swipes their card, and the transaction is applied to the wrong plan (for example, the transaction was applied to their FSA instead of their HRA for a similar expense), you must complete and submit a Transaction Reversal Request Form

Once you submit the form, our Claims team will process the information, reverse the incorrect transaction, and apply the transaction to the correct plan or plan year.
