Name Change

If you have experienced a change to your legal name, these steps will help you to update your name in our system.

You can quickly and easily submit a name change request online via your Participant Portal. To change your name in our system, follow these steps:

1. Log into the Ameriflex Participant Portal.

2. After logging in, select the More dropdown menu at the top of the screen, then select Profile.  

3. In the Family section on the right side of the page, click the Edit button next to your name.

4. On the new information screen that appears, enter your new name in the appropriate fields.

5. After you have entered your new name, you will be prompted to upload legal documentation to show proof of your updated name. Choose the Upload Files button at the bottom of the information screen to submit this documentation.

6. Once you have verified that all information is correct and have uploaded documentation, click the green Save Changes button at the bottom of the screen.

7. Please allow 3-5 business days for the documentation that you submitted to be reviewed. If approved, the changes will appear on your account. If additional or corrected documentation is needed, we will contact you for additional information.
