Cardholder point of sale transaction dispute

Cardholder point of sale transaction dispute forms can be completed if a participant would like to dispute debit card activity. Dispute forms must be submitted within 90 calendar days of the transaction date. If the dispute is submitted over 90 days, the dispute will be reviewed, but there is no guarantee the transaction will be resolved in favor of the participant.

Steps:

  1. Access the Cardholder Dispute form
  2. Complete the form in its entirety.
  3. Once completed, email the form to service@myameriflex.com.
Note: Please be sure to follow the general guidelines as noted on the dispute form. Your form must include all of this information to complete a thorough review.
