Fees that apply to the participants’ accounts

When participants receive their Health Disclosure from Ameriflex, they will need to e-sign the document. This document will outline all current fees associated with the account. The fees are listed below for your reference as well. 

  • $25 fee deducted from balance if a participant chooses to close their HSA
  • $2.50 per month if a participant drops below $1,000 for investment
  • $1.95 per month if the participant wants to remain with paper instead of electronic statements. Participants are able to change their delivery preferences for this through their Ameriflex account by going to HSA Account Details, Statements and then Delivery Preferences.
  • $8.00 fee for ordering new checks. The order form for checks can be found by clicking here.
