Depending on the custodian bank holding your assets, you have two options for transferring your Ameriflex Health Savings Account. You can download the Ameriflex Health Savings Account transfer form, fill it out, and send it directly to your new custodian bank along with your transfer request. Alternatively, you can obtain a new vendor transfer request form from your new bank and send it back to Ameriflex for processing.

Please be aware that all transfer requests may take up to 4-6 weeks, depending on the processing time with your custodian bank. For more information on transferring your HSA funds, please contact the Ameriflex Participant Services team.
