The Life Event form for a Coverage Change also initiates the process to terminate a DCA that may have been enrolled in error.
Terminating a DCA account will also be completed from the QLE Coverage Change Form.

1. Choose the “Change in employee’s benefit status” Life Event Type.
2. Once you have completed the Lookup process, use the Terminate Coverage Action Type. If you need a Payroll Refund, use this form to provide the GIC Coordinator with that information.
